Yes, I've had insured AND registered envelopes dumped in my mailbox - no
signature to acknowledge receipt, nothing! This was "thanks" to the Coconut
Grove Post Office. (I had several other problems with the same P.O branch.
After several visits, I finally complained to the Postmaster General in
writing. Result was our letter carrier was either switched to a different
    route, or fired. )
I figured that if something "vanished" it would be my problem, which is why
I got myself a PO Box in a different part of Miami. Never had a problem
    since.
In Miami, there is a 3-year waiting list to get a job with the USPS. They
    don't need to employ morons!
Its a good point about UPS insured - there's no hassle, from what I've
    heard.
FEDEX, by the way, will not accept coins for shipment.
